About Evoke Strong PreventT2 Lifestyle Change Program:

The lifestyle change program helps people with prediabetes prevent or delay type 2 diabetes. Type 2 diabetes is a serious disease that affects the lives of millions of people and their loved ones (2019 statistics show about 35 million U.S. adults have type 2 diabetes and 96 million have prediabetes).

We want to help you prevent it!

Evoke Strong PreventT2 lifestyle change program brings people together to learn skills for living a healthier life.  A lifestyle coach provides the guidance to show you how to reach or stay at a healthy weight, add more physical activity into your day, make healthy food choices, and manage stress.

Your First Step

One in three American adults has prediabetes.  Without lifestyle changes, 15 to 30 percent of people with prediabetes can develop type 2 diabetes within five years.

Most people with prediabetes don’t have visible symptoms.  So the first step is to find out if you’re at risk.

✔︎ Are you 45 years old or older?

✔︎ Are you overweight?  (BMI greater than 25; if Asian, BMI greater than 23)

✔︎ Are you physically active less than 3x a week?

✔︎ Do you have a family history of type 2 diabetes?

If you answer yes to two or more questions, you might be at risk for prediabetes.  Take the prediabetes risk test and talk to your healthcare provider about your results.  Your provider may recommend a blood test for prediabetes.  Only a blood test can confirm if you have prediabetes.

Your Health is worth the Investment - take the next step!

If you have prediabetes, your next step is to fill out an interest form to verify your eligibility and express your level of commitment to the program.  We’ll be in touch with you shortly.  Thank you!

What you don't know can hurt you! Take the Prediabetes Risk Test

1 in 3 American adults has prediabetes and most don’t know they have it.  Take the test to discover your risk.

Note:  If the test indicates you’re at a high risk and are likely to have prediabetes, ask your doctor for a blood test.   The good news is prediabetes can be reversed.  When you start making healthy lifestyle changes,  it’s highly likely you will prevent type 2 diabetes.
